Home care startup cost categories, explained
Startup costs fall into six groups: getting legal, protecting the business, tools, people, marketing, and cash reserves. Each has different drivers. The only reliable way to price them is to get written quotes in your own state, then enter them in the calculator.
1. Getting legal
Entity formation, state application or license fees, and legal review of your documents. State fees and timelines come from your state agency, so look up the current schedule there rather than trusting an old blog post.
Questions to ask
- Which license category fits the services I will provide?
- What is due at application, and what is due after approval?
- What must be in place before the agency can accept a client?
2. Protecting the business
Liability insurance, workers' compensation, and a bond where required. Premiums depend on your payroll, services, and insurer. See insurance, bonding, and legal costs.
3. Tools
Scheduling and billing software, phones, a website, and equipment. See software, office, and equipment.
4. People
Background checks, screening, training, onboarding, and the pay for your first office hire if you make one. Caregiver payroll itself belongs in the cash reserve, not here.
Questions to ask
- What training does my state require before a caregiver can work?
- Who pays for screening, me or the candidate?
- Will I hire employees, and what does that change for insurance and payroll?
5. Marketing and recruiting
You need both clients and caregivers. Budget for both, and expect recruiting to be continuous rather than a launch expense.
6. Cash reserves
The largest and least visible cost. Size it with the cash reserve calculator. If you want a full plan around these categories, Home Care Agency Blueprint covers it in its paid program.
Costs vary by state. For state licensing requirements and fees, see the state licensing guides at starthomecareagency.com. We do not quote state fees on this site.

Common questions
Which category do new owners underestimate most?
Cash reserves. Payroll goes out on a fixed schedule while payment comes in later.
How do I get accurate quotes?
Ask at least two vendors or insurers for written quotes using the same assumptions about payroll, hours, and services.
